I do remember a psychiatrist once asking me if the feelings I got from self harm were anything like an orgasm, and there are some similarities I suppose so maybe your wanking simile is quite a good one Felix.

For some reason I hadn’t considered the role endorphins and adrenaline play when your body is damaged. I think perhaps I was focusing too much on where the thought itself comes from rather than the effect achieved by following it through. Interesting.

Curious stuff. My first guess - and I'd imagine this has been said before by other people, since it's my first guess - is that cutting oneself provides some adrenaline followed by some endorphins, and is rather like smoking a fag or wanking.
